This where and mechanical-electro-optical scanner would fix things, ScanTron to bring back a familiar name.

Accuracy can be tested with a test run. An issues with a batch of ballots can be run again, and if results are different a different machine can be used. Since there is no software to hack, a physical examination of the machine will reveal tampering. And if the machines are access control, who had access is known.
